Customer/Client Agreement

Dear Valued Customer/Client,

My name is Juline Marie Box, and I thank you for choosing Out-Of-The-Box Teaching & Tutoring (OOTB) for your learning needs. By utilizing OOTB products and services, you are indicating that you understand and agree to the following Terms of Service:

DESCRIPTION OF SERVICES

OOTB provides online teaching and tutoring services to young people and their families. The OOTB website provides information about OOTB products and services and facilitates clients' booking of and payment for OOTB services. Through the OOTB website, clients and other customers can also make donations to the OOTB HOPE Scholarship fund, as well as purchase OOTB gift certificates, packages, subscriptions, courses, merchandise, and other products, as these products and services become available.

SUPERVISION OF SESSIONS FOR CLIENTS UNDER THE AGE OF 18

During the online teaching/tutoring sessions, the presence of parents/guardians for people under the age of 18 is both welcomed and required for the purpose of:

1) Teamwork: The student, parent/guardian, and tutor/teacher are all on the same team, working together to help the student grow, learn and excel;

2) Transparency: Parent/guardians have the primary responsibility to teach and train their child, and I want parents/guardians to know what and how I am teaching their child;

3) Open communication: While the session itself will generally be led by me (Juline), the student and his or her parent/guardian are welcome to ask questions, share concerns, and/or suggest ideas both before and after the session.

4) All parties’ safety: In order to have an effective and engaging session, all parties (student, parent/guardian, and teacher/tutor) need to be in an environment where they feel safe, supported, and engaged.

Mandatory Reporting

As a former public school teacher, I was a mandatory reporter both inside and outside of school. Mandatory reporters have a legal obligation to report known or suspected abuse or neglect relating to children, elderly persons, dependent adults, and people with disabilities. As the sole teacher/tutor of Out-Of-The-Box Teaching & Tutoring, I have chosen to maintain this status of being a mandatory reporter. This means that, if a child or adult using my services discloses information to me that I would be legally required to report as a public school teacher, I am going to report that information to the appropriate authorities in this role of Out-Of-The-Box teacher/tutor as well.

PAYMENT

The initial 30-Minute Consultation for all services is free of charge, so payment information is not required when booking this consultation. Full payment for all other services (tutoring sessions, service packages, courses, etc.) is due at the time of booking the appointment(s). Full payment for other available products (gift certificates, subscriptions, merchandise, etc.) is due at the time of purchase.

CANCELLATION/REFUND POLICY

There may be occasions when you need to cancel or reschedule your appointment. To ensure the smooth operation of OOTB and provide the best service to all OOTB clients, the following appointment cancellation policy is in place:

Cancellation Notice:

OOTB requests a minimum of 24 hours of advance notice for any appointment cancellation or rescheduling. Failure to provide sufficient notice results in a cancellation fee.

Cancellation Fee:

If you fail to provide the required 24 hours’ advance notice for appointment cancellation or rescheduling, only 50% of the appointment’s cost will be credited back to you.

Rescheduling:

If you wish to reschedule your appointment, OOTB will do its best to accommodate your request based on availability. Rescheduling requests made with less than 24 hours notice may be subject to the cancellation fee.

No-Shows:

If you fail to show up for your scheduled appointment without any prior notice, you will be considered a “no-show.” No-shows will result in a 50% cancellation fee for that appointment and may affect your ability to schedule future appointments.

Other Circumstances:

OOTB understands that unforeseen circumstances may arise, and OOTB will make exceptions for emergencies or unavoidable situations. In such cases, please contact OOTB as soon as possible to discuss your situation and potential alternatives.

Communication:

To cancel or reschedule your appointment, please use the booking confirmation email you received at the time of booking your appointment. Otherwise, you are welcome to email or call me using the contact information provided below.

I recommend that you obtain a confirmation of your cancellation or rescheduling from me (Juline) to ensure proper record-keeping.

Please note that this policy is in place to ensure fairness to both OOTB valued clients and me at OOTB. By booking an appointment with OOTB, you acknowledge and agree to adhere to this cancellation policy.

THANK YOU

I appreciate your understanding and cooperation to make your learning experience enjoyable, effective and efficient. If you have any questions or concerns regarding this Customer/Client Agreement, please don’t hesitate to contact me.

Thank you,

Juline Marie Box
